...Because the person is just ill-suited for the role, and how much because of what a person is going through in his/her personal life at that point of time.
...Because the person can't stand the people he/she works with, and how much because of the actual responsibilities of the job.
...Because he/she doesn't understand what exactly needs to be done, versus how much because it is really intellectually mind-numbing.
...Because he/she lacks the choice to work in his/her preferred method, and how much because the company's method is just plain ridiculous.
...Because the person feels stuck in the role as a result of a lack of choices outside, and therefore feels like they are compromising.
...Because the person really has no idea what his/her calling in life is, and is in the current role simply as a life- and time-filler.
...Because he/she desperately needs the money and therefore, is forced to do this for a living, as opposed to doing this out of passion.
...Because the name of the company the person works for is not as high up in the ladder as they'd like it to be.
...Because of how much money a person's peers in similar roles/positions are making, and how far ahead in the organisation people have gotten as opposed to them.
...Because of how little time it leaves a person to pursue their dreams/passions and to live their personal lives separately from their professional ones.
...Because of the lethargy that sets in from being in the comfort-zone for way too long.
...Because of the person not accepting the fact that the he/she lacks what it takes to do the job well.
...Because the person truly feels they could be doing much, much better.
...Because the person mistakenly believes they could be doing much, much better.
...Just because of the job description itself.

God! Awesome post... so many of your points ring true! :( [ I need help, methinks :) ] What resonate most, though, are these:
a. The one about jobs as time-fillers and lack of knowledge of one's true calling
b. The one about working for money as opposed to working for passion
c. The one about no time for a fulfilling personal life
